Treatment-Resistant Depression: A Different Approach

When depression doesn’t improve despite trying traditional treatments, it can be frustrating and discouraging. At Appleton Mental Health, we understand that treatment-resistant depression may require a different perspective and a more individualized approach.

A Different Approach to Depression Care

Our approach to treatment-resistant depression focuses on understanding the whole person—not just the diagnosis. We take the time to review your symptoms, treatment history, medications, lifestyle factors, and individual goals to develop a personalized plan of care.

If you have tried one or more antidepressants or other treatments without experiencing the improvement you hoped for, that does not mean you have run out of options. There may be additional treatment strategies worth considering.

Our evaluation may include:

-A comprehensive review of your previous depression treatments and responses
-Assessment of current symptoms and functional impact
-Review of medications, potential side effects, and treatment considerations
-Evaluation for factors that may contribute to persistent or difficult-to-treat depression
-Discussion of evidence-based treatment options that may be appropriate for your individual situation
-Ongoing monitoring and adjustments based on your response and treatment goals

You Deserve a Personalized Plan

Treatment-resistant depression is not the same for everyone. What works for one person may not work for another. We believe effective care begins with listening, understanding your history, and identifying a treatment strategy that fits your unique needs.

At Appleton Mental Health, our goal is to help you move beyond simply managing symptoms and toward meaningful improvement in your quality of life.

If traditional approaches have not provided the relief you were hoping for, a different approach may be worth exploring.

Treatment recommendations are individualized and depend on a comprehensive clinical evaluation. Not every treatment option is appropriate for every patient, and results may vary.

What Is SPRAVATO®️?

SPRAVATO®️ (esketamine) nasal spray is a prescription treatment used for certain adults with treatment-resistant depression (TRD). It offers a different approach for people who have not experienced adequate improvement with traditional antidepressant treatments.

Unlike conventional antidepressants, SPRAVATO®️ works through the brain’s glutamate system, specifically targeting NMDA receptors. This provides a different mechanism of action than many commonly prescribed antidepressants.

A Different Approach for Treatment-Resistant Depression

At Appleton Mental Health, we recognize that depression does not affect everyone in the same way—and sometimes traditional treatment may not provide the relief you need. SPRAVATO®️ may be an option for appropriate patients following a comprehensive evaluation.

Treatment with SPRAVATO®️ is administered as a nasal spray under the supervision of a qualified healthcare professional. Because SPRAVATO®️ can cause temporary side effects such as sedation, dizziness, or changes in perception or blood pressure, patients are monitored during treatment and for a period afterward.

Could SPRAVATO®️ Be Right for You?

If you have been diagnosed with treatment-resistant depression and have not achieved adequate relief from previous antidepressant treatments, you may benefit from an evaluation to determine whether SPRAVATO®️ is an appropriate option.

At Appleton Mental Health, we take a personalized approach to care. We will review your medical and psychiatric history, previous treatments, current symptoms, and treatment goals to determine the most appropriate options for you.

You don’t have to assume that your current level of depression is as good as it gets. A different approach may be possible.

What to Expect During SPRAVATO®️ Treatment

At Appleton Mental Health, we believe your comfort and safety are an important part of every SPRAVATO®️ treatment session. Our team provides a calm, supportive environment while carefully monitoring you throughout your visit.

Before Your Treatment

Before SPRAVATO®️ is administered, we will review how you are feeling, answer any questions, and obtain your vital signs.

As part of our safety monitoring, we check your:

  • Blood pressure
  • Oxygen saturation
  • Other vital signs as clinically appropriate

Because SPRAVATO®️ may temporarily increase blood pressure and can cause changes in alertness or respiratory status, monitoring these vital signs helps our clinical team assess your response to treatment and determine when it is appropriate for you to leave. The FDA prescribing information recommends blood-pressure assessment before treatment and reassessment after administration, as well as respiratory monitoring with pulse oximetry during the post-dose observation period.

Preparing for Your Appointment

To help reduce the likelihood of nausea or vomiting during treatment, the SPRAVATO®️ prescribing information recommends that patients avoid eating for at least 2 hours before administration and avoid drinking liquids for at least 30 minutes before administration.

Appleton Mental Health asks patients to avoid drinking for 1 hour before administration as part of our office preparation instructions.

These guidelines are intended to help minimize the possibility of nausea or vomiting during your treatment session.

Please follow any additional instructions provided by your Appleton Mental Health treatment team.

Administration of SPRAVATO®️

SPRAVATO®️ is administered as a nasal spray under the direct supervision of a healthcare professional. You will remain at our office throughout the treatment and observation period.

After administration, you may experience temporary sensations such as:

  • Sleepiness or drowsiness
  • Dizziness or lightheadedness
  • Feeling relaxed or detached
  • Changes in perception
  • Temporary changes in blood pressure
  • Nausea

These effects can vary from person to person. Our team will be available throughout your session and will monitor you for any concerning symptoms.

Vital Sign Monitoring During Your Session

Your safety is our priority. At Appleton Mental Health, we monitor your blood pressure and oxygen saturation throughout the treatment process.

Vital signs are obtained:

1. Before SPRAVATO®️ administration
We establish your baseline blood pressure and oxygen saturation before treatment.

2. Approximately 45 minutes after administration
We reassess your vital signs to monitor your response to treatment. This timing is consistent with the period when blood-pressure effects are important to assess; the FDA labeling specifies reassessment at approximately 40 minutes.

3. Before your session is completed
We perform a final assessment to make sure you are clinically stable before you leave our office.

Oxygen saturation is monitored with pulse oximetry during the observation period, consistent with FDA safety guidance for SPRAVATO®️ treatment.

Your Recovery and Observation Period

Following administration, you will remain in our office for observation. You will have a comfortable space where you can relax while our clinical team monitors your response.

SPRAVATO®️ must be administered under the direct supervision of a healthcare provider, and patients are monitored following administration. The FDA labeling recommends monitoring for at least 2 hours, with additional monitoring when clinically indicated.

You should plan to spend approximately 2 hours or more at the office, depending on your individual response and your provider’s assessment.

Transportation Home

Because SPRAVATO®️ may temporarily affect alertness, judgment, coordination, and perception, you should not drive or operate machinery after treatment. You will need to arrange for a responsible adult to drive you home after your session.

Your provider will let you know when your monitoring period is complete and when you are appropriate for discharge.

A Supportive, Monitored Approach

At Appleton Mental Health, we understand that beginning a new treatment can feel uncertain. Our goal is to provide individualized care in a supportive environment while carefully monitoring your response during each session.
